Pear Joseph is a gender non-conforming gay man whose story speaks directly to one of Europe's most contested clinical questions: could young people who grow up outside gender norms be steered toward a transgender pathway that isn't right for them? His writing and advocacy have given that question a personal face and a political edge.
Pear Joseph grew up as a gay boy who did not conform to masculine expectations. In an era when gender non-conformity in children is increasingly read as a signal of a transgender identity, his experience raises an urgent question that European health systems are only beginning to confront: what happens to the gay kids who simply do not fit the masculine mould?
His widely read essay “Not Being Affirmed Saved My Life” made the case that being steered away from a gender transition — rather than toward one — was what allowed him to arrive at a settled, happy identity as a gender non-conforming gay man. Through his work with the LGB Courage Coalition, he has become a prominent voice arguing that affirmative-care models risk erasing a generation of gender non-conforming LGB young people.

For European audiences, his perspective lands in the middle of live policy debates. Sweden and Finland have already restricted gender-affirming interventions for minors following systematic evidence reviews, while the Netherlands — home to the original Dutch Protocol — is reassessing its own approach. France and Germany are still writing workable clinical guidance. Joseph’s personal testimony gives human texture to a consistent research finding: that a significant proportion of gender non-conforming children, if supported to wait, grow up to identify as lesbian or gay rather than transgender.
